In 2005 Davol, Inc. announced a recall on hernia mesh patch lot numbers 0010206, 0010207, 0010208, 0010209, 0010202 and 0010204 for their Kugel hernia mesh used in hernia repair surgery. This hernia mesh recall was later expanded in January 2007 to include additional product codes and lot numbers.
Here is a timeline of major events in the Kugel hernia mesh recall:
February 2003: Davol started to receive complaints about Kugel mesh patch ring breaks in their extra large hernia mesh.
May 2004: Davol sets up complaint handling system.
September 2004: Testing reveals manufacturing problem with a welding process.
December 2005: Voluntary recall of Extra Large Oval Composix Kugel Mesh patches.
January 2006: FDA investigation
March 2006: Original Composix Kugel hernia mesh recall is extended to oval, large oval and large circular patches.
March 24, 2006: Letters are sent to hospitals and doctors notifying them of the expansion of the original recall. Doctors are also informed about the risk of the welds breaking and resulting complications such as bowel perforations and chronic enteric fistulas.
January 2007: The Kugel mesh recall is expanded again to oval, large oval and large circle Composix Kugel patches manufactured before October 2005.
